Innovative New Service Streamlines Mental, Physical Health Care for Seniors

Since 1893, Henry Street has been a force of innovation in social services and health care, and the Settlement is now offering another pioneering service: Starting this month, a new Geriatric Care Program (made possible by a grant from the New York State Office of Mental Health) has allowed Henry Street's Senior Services and Primary & Behavioral Health divisions to collaborate in offering comprehensive care for seniors.
Seniors in Settlement programs can be referred to the Geriatric Care Program's full-time coordinator, who will assess their needs and meet with them regularly to ensure they receive necessary care, which could include physical and mental health treatment, home-delivered meals, and social and recreational activities at the Settlement's Good Companions Senior Center. The new program removes hurdles for senior clients, allowing them to navigate these services easily.
Henry Street's Geriatric Care Program is part of New York State's Home Health Care Model a first-of-its-kind, holistic approach to medical care.
